If you have been in contact with someone who has tested positive for COVID-19 and would like to self report, please click the COVID Exposure Questionnaire link below and complete a few quick questions that help us determine when you can be released from quarantine. According to the Center for Disease Control a close contact is considered someone that has been within 6 feet of a COVID positive individual for 15 minutes or more throughout the course of a day. If you are fully vaccinated you do NOT need to quarantine per CDC recommendations, but you do need to monitor for symptoms.
CDC recommends a 14-day quarantine. We understand this is a burden for some households, therefore following a shortened quarantine of 10 days is an acceptable option. Quarantine can end after Day 10 without testing and if no symptoms have been reported during daily monitoring. When ending quarantine after day 10, masking is necessary when in public for days 11-14. Continue to monitor symptoms for the full 14 days.
