Food Establishment Plan Review Application

This form is to be filled out and submitted prior to the opening of a business that handles food in any manner. Health permits are required for all food facilities.

Animal Occupancy Permit Application

This form is to be filled out and submitted prior to the opening of a business that handles, shows, or brings animals to the public in any manner. Health permits are required for all such facilities.

Frozen Dessert License Application

Any establishment wishing to sell frozen desserts to the public must keep and maintain a Frozen Dessert License from the State of Missouri. This is in addition to a Health permit from Taney County Health Department.

Tattoo and Body Art Establishment Application

Any establishment practicing Body Modification, including tattooing or piercing inside the city limits of Branson, Missouri must apply for and maintain a current TCHD health permit in addition to City and State permits to practice their trade. This form must be filled out and submitted prior to opening your doors to the public.

Temporary Events

Temporary Food Event Organizer Application

This form is used by an organizer of a temporary event in which there will be multiple vendors. It must be on file 30 days prior to the event.

Temporary Tattoo Establishments at Special Events

The state of Missouri requires a notarized application to be submitted thirty (30) days prior to the event, along with all appropriate fees per event by the licensed artist. The license is in effect for fourteen (14) days.

The Taney County Health Department requires a copy of the temporary license issued by the State of Missouri and the purchase of a temporary Taney County Health Permit prior to the scheduled event.
