WHAT IS CASE MANAGEMENT?
Case management is a service offered to pregnant women on Medicaid
who might have a potential risk effecting the health of mother or
baby. Examples of a risk are:
- 17 years
or younger
- smoking
- if you have
two pregnancies less than a year apart
- history
of premature delivery
- first pregnancy
Our Goal: to help you have a healthy pregnancy and baby
by:

a. Stages of
b. What to expect
c. How your baby is developing

Ways to promote a healthy pregnancy
a. Early prenatal care
b. Eating right
c. What can cause harm to unborn baby

Refer you to other services that can help with your social,
financial, and educational needs.
How often Do I See You?
Usually I'll see you once a month. We try to schedule these the
same day of your WIC appointment. Then our last visit is done at
your home after the baby's born. We will then discuss baby care,
baby shots, well baby checkups, and family planning, along with
much more.
